need help with direct debit form

I have a direct debit form from general des eaux (water company) and

at the bottom it says;

fait a ........................... le............................. signature:

I know I have to sign and date this but which way round

fait a .Rouen.......................... le...2me Juin 2005......................... signature: Fred Smith

I've been told by bank tellers that I don't need to enter the location but always do "just in case".

Fait a:  Write the name of the town where you are at the time you complete the form.

Le: Write the date

Signature: Sign it
