Sunday Driver Posted May 26, 2011 Share Posted May 26, 2011 Not quite sure what you mean by the form "Declaration Medical".If you mean the section of the driving licence application form that is 'Réservé a la Commission Médicale' and completed by the French doctor authorised to carry out medical examinations, then the answer is no.....
