Question, can I assume they have filled in all that needs to be filled in, and all I have to do is fill in my address, ss no, and sign it, (which I can do OK), and get it to CPAM. Or will there be something I should be ticking before I send it? Only the second time I've had one, and the first one was when I signed on at the doctor's before my carte vitale arrived, and he filled it all in for me. Will Posted September 11, 2009 Share Posted September 11, 2009 That's basically it - name and SS number are the important things and although it isn't normally necessary to fill in the address unless it is different from that which the primary health cover provider has on its records it does no harm to complete it. The only other thing to remember is that copies of prescriptions are often required - so if you were given one for your scan then send the dupliacte, or a photocopy.
